Background and Aims:

Regional analgesia has become the preferred technique because general anesthesia has been associated with higher maternal mortality. Spinal analgesia has many advantages including being a single injection, faster and easier.

Methods:

This study was carried out on forty parturient scheduled for cesarean sections at El-Shatby Hospital
- Group I [Fentanyl group]: 20 parturient received spinal analgesia consisting of 2 ml of 0.5% hyperbaric bupivacaine and 0.5 ml fentanyl 
- Group II [Clonidine group]: 20 parturient received spinal analgesia consisting of 2 ml of 0.5% hyperbaric bupivacaine and 0.5ml clonidine 

Results:

• The results revealed that parturient received intrathecal clonidine had a higher and faster level of sensory blockade and delayed regression of sensory level compared to whom received intrathecal fentanyl.
• Sedation was significant in clonidine group, although it did not occur in any parturient of fentanyl group and almost happened in 25% of the parturient.
• Nausea, vomiting and itching were more evident in parturient received intrathecal fentanyl than who received intrathecal clonidine.     

Conclusions:

• Using intrathecal clonidine for cesarean section showed better sensory blockade and early recovery of motor blockade as compared to intrathecal fentanyl.
• Intra operative sedation was significant side effect in clonidine group. But this side effect add  advantage to clonidine group over the fentanyl group.
• Fewer requests for supplementary analgesia in clonidine group in post-operative period.
• Less incidence of nausea, vomiting and itching in clonidine group than fentanyl group.
