ADDUCTOR CANAL BLOCK EFFECTIVENESS COMPARED WITH FEMORALNERVE BLOCK AFTER PRIMARY KNEE ARTHROPLASTY

Background and Aims:

To compare which of the regional anaesthesia methods: continuous adductor canal block or continuous N. Femoralis block can manage postoperative pain more efficiently.


Methods:

Prospective randomized trial included patients undergoing elective knee replacement. I group received Adductor canal block (AB) with 48 hours continuous postoperative infusion with local anesthetics. II group received N. Femoralis block (NF). Postoperative pain relief was provided by multimodal analgesia and morphine as rescue analgesic. Pain intensity at rest and movement, M. Quadriceps femoris strength, additional morphine were measured. Patients fulfill questionnaire about satisfaction.

Results:

 Pain at rest, bending and walking didn’t differ between two groups before surgery. Pain at rest POD1, POD2, POD 3 didn’t differ much. Pain in the knee joint movement POD1 POD2, POD3 was similar. Total morphine consumption AB 25,93 ± 11,3 mg, NF 32 ± 23,1 mg. 

Conclusions:

Adductor canal block reduce pain after total knee replacement as potent as N. Femoralis block according to postoperative pain and morphine consumption. However, there is a tendency that the adductor canal block group has less pain and reduced morphine consumption.
